To obtain the code, please faculty must sign in to the MySuffolk portal. Go to Committees > My Committees > SBS Faculty Resources > Shared Files > OTM Resources . Look in the PDF file called Qualtrics> Qualtrics.pdf. The PDF includes self-enrollment codes for both faculty and students.
